本校將於 十月二十一日星期五(上午 8:00至 10:00)

         實施 1-6年級學生暨教職員工流感疫苗注射

       地點:簡易禮堂

  1. 教職員工截止報到時間 10:00，攜帶健保卡，隨到隨打(簡易禮堂找黃莉雯護理師報到+領取評估單，班級導師隨班接種)。
  2. 班級請依排定時間表，避免現場等候太久，請在教室等待電話通知
  3. 轉發流感接種通知單請學生貼在聯絡簿(只須發給同意接種學生且有交同意書)—已經放在辦公室班級抽屜櫃
  4. 預防注射當天請老師事先檢查聯絡簿確認家長是否有其他交代事項，流感疫苗注射當天會有醫師到校評估是否適合接種疫苗，當日未接種疫苗學童請持補種單自行預約接種可至 13區衛生所(桃園市衛生所免費補種 3791888)或持補種單自付掛號費與診察費於醫療院所補種。
  5. 防注射當日流程，請老師務必檢查學童聯絡簿如家長臨時不同意接種或原本調查已不同意接種之學生請留在教室
  6. 由老師統一帶隊至簡易禮堂集合(請由 205樓梯)，並協助發放同意書暨接種後注意事項單子(請核對學生)
  7. 醫療院所協助測量體溫，學生持自己的接種單依序排隊，接種完畢由 208樓梯離開簡易禮堂，等全班接種完畢再一起回教室，須在教室內休息 30分鐘，請勿下課，如學生有身體不適請導師務必親自陪同學生至簡易禮堂，請醫師評估學生健康狀況，當天請勿做激烈運動，體育課請調整靜態課程，預防注射後且多喝水，若返家接種後有身體不適如發燒超過 38 度請至醫療院所診治並通報健康中心分機 119 。

8.10/21星期五還在居隔教職員工生不能到校接種，會發補種單
