一、依據高級中等以下教育階段非學校型態實驗教育實施條例及本局 109 年 9 月 25 日桃教小字第 1090088845 號函（諒達）辦理。


二、本市 109 學年度第二階段辦理「高級中等以下教育階段非學校型態實驗教育」採線上申辦，請各受理收件學校於 109 年 11 月 1 日至 11 月 7 日止登錄「桃園市高級中等以下教育階段非學校型態實驗教育申請暨審議作業系統（ https://tycnee.psees.tyc.edu.tw ）」，將申請人送交之申請書掃描上傳至線上申請系統，並進行申請案資料檢核，檢核注意事項如下：
(一)申請人需繳交資料是否已全數上傳完成。
(二)申請書是否為法定代理人親筆簽名。
(三)需為最近 3 個月內之戶口名簿或戶籍謄本並含詳細記事。若無戶口異動，可另附線上查詢截圖證明即可（中華民國內政部戶政司全球資訊網/戶口名簿請領記錄查詢 https://www.ris.gov.tw/app/portal/581 ）。
(四)學生申請之年級是否正確。
(五)國民中小學教育階段擬部分課程、資源或社團等項目與設籍學校合作者，需於實驗教育計畫內檢附與學校協調後之相關文件。
(六)高級中等教育階段擬與本市轄屬高中職合作者，需於實驗教育計畫內檢附雙方用印之合作計畫書（倘高一新生於放榜確定合作學校，應由合作學校在開學前兩週函報其合作計畫等相關資料至本局）。
(七)經本局許可參與非學校型態實驗教育且擬自 109 學年度同時取得教育部主管高級中等學校學籍之學生，應依高級中等學校多元入學招生辦法之規定入學，並由其法定代理人就課程與教學之實施、成績之評量、校內活動之參與、學雜費之收取及其他有關實驗教育之事項，與該學校擬訂合作計畫，經學校函報教育部許可後進行合作。
