主旨： 檢送「桃園市高級中等以下教育階段非學校型態實驗教育成果報告書（應屆畢業生）」及「桃園市高級中等以下教育階段非學校型態實驗教育學生學習狀況報告書（非應屆畢業生）」撰寫格式各 1 份，請轉知家長知悉，請查照。
說明：
一、 依據「桃園市 114 學年度高級中等以下教育階段非學校型態實驗教育工作實施計畫」辦理。
二、 查「高級中等以下教育階段非學校型態實驗教育實施條例」第 20 條規定：「（第 1 項）辦理個人實驗教育者，應於每學年度結束後 2 個月內，提出學生學習狀況報告書，為期 3 年以上之實驗計畫於計畫結束當學年應併提出實驗教育成果報告書……。（第 2 項）辦理團體實驗教育及機構實驗教育者，應於每學年度結束後 2 個月內，為期 3 年以上之實驗計畫於計畫結束當學年應併提出實驗教育成果報告書……。」。
三、 依上開條例規定，非學校型態實驗教育學生學習狀況報告書（或成果報告書）之繳交時間訂於 9 月份；惟考量國小 6 年級、國中 9 年級及高中 3 年級之學生畢業所需，爰繳交時間為 4 月份，俾利取得畢業證書或完成實驗教育證明。
四、 旨揭學生學習狀況報告書（或成果報告書）審查作業收件時間如下：
(一) 國民教育階段：
１、 國小 6 年級及國中 9 年級學生：自 115 年 4 月 11 日起至 115 年 4 月 17 日止。
２、 國小 1 年級至 5 年級及國中 7 年級、 8 年級學生：自 115 年 9 月 1 日起至 115 年 9 月 7 日止。
(二) 高級中等教育階段：
１、 高中 3 年級學生：自 115 年 4 月 11 日起至 115 年 4 月 17 日止。
２、 高中 1 、 2 年級學生：自 115 年 9 月 1 日起至 115 年 9 月 7 日止。
五、 本案請協助轉知申請人依限將學習狀況報告書（或成果報告書）電子檔上傳至「桃園市高級中等以下教育階段非學校型態實驗教育申請暨審議作業系統」（網址： https://tycnee.psees.tyc.edu.tw/）。
